Experimental drug combo postponed resistance to therapy by 4 months, study showed.

SATURDAY, Sept. 29 (HealthDay News) -- Researchers say they've discovered a two-drug combination that delays treatment resistance in patients with advanced melanoma.

By targeting different points in the same growth-factor pathway, the kinase inhibitor drugs dabrafenib and trametinib postponed the development of drug resistance in patients with BRAF-positive metastatic melanoma, the study authors said.

Melanoma is the most serious, and often deadly, form of skin cancer. In about half of patients with melanoma that has spread, tumor growth is caused by genetic mutations that keep the BRAF protein -- part of the MAPK cell growth pathway -- constantly activated. Drugs that inhibit BRAF activity can rapidly stop and reverse tumor growth in about 90 percent of patients. But the response is temporary in most cases, and tumor growth resumes in six or seven months, the researchers explained.

Previous research suggested that this drug resistance develops because the MAPK pathway gets turned back on through activation of MEK, another protein that is part off the MAPK pathway.

"We investigated this (drug) combination because of research we and others have conducted into the molecular underpinnings of resistance to BRAF inhibitor therapy," study lead author Dr. Keith Flaherty, of the Massachusetts General Hospital Cancer Center, said in a hospital news release.

The phase 1 and 2 study was sponsored by GlaxoSmithKline, which developed both drugs.

"We found that adding the MEK inhibitor trametinib to BRAF inhibitor dabrafenib clearly delays the emergence of resistance. In fact, the combination was at least twice as effective as BRAF inhibition alone," he said.

One expert agreed that the results were encouraging.

"This study addresses the problem of resistance to the mitogen-activated protein kinase [MAPK] pathway. This study combined dabrafenib [a selective BRAF inhibitor] and trametinib, [a selective MAPK kinase (MEK) inhibitor]. These drugs can be safely combined, and the survival was significantly improved without untoward side effects," said Dr. Michele Green, a dermatologist at Lenox Hill Hospital in New York City.

The study was conducted at 14 sites in the United States and Australia and included 162 patients who received different dose combinations of the drugs: two daily 150 milligram (mg) doses of dabrafenib plus one 2 mg dose of trametinib; the same dabrafenib dose with a 1 mg dose of trametinib; or treatment with dabrafenib alone.

Patients who received dabrafenib alone were able to receive the full-dose combination treatment if their cancer resumed progression.

Treatment with both combinations of dabrafenib and trametinib led to a four-month longer delay in drug resistance than treatment with dabrafenib alone. After one year of treatment, 41 percent of patients receiving the full-dose combination treatment had no progression of their melanoma, compared with 9 percent of those receiving dabrafenib alone.

Among patients receiving the combination treatment, the development of side effects such as skin rash and a less dangerous type of skin cancer called squamous cell carcinoma was similar to that typically seen in patients taking only one of the drugs.

The study was scheduled for presentation Saturday at the European Society for Medical Oncology meeting in Vienna and simultaneous publication in the New England Journal of Medicine.

The drug combination is now being tested in a larger phase 3 study, which is required for U.S. Food and Drug Administration approval.

The toll has climbed steadily in recent months with a spate of attacks by Afghan army and police ? supposed allies ? against American and NATO troops. That has raised troubling questions about whether countries in the U.S.-led coalition in?Afghanistan?will achieve their aim of helping the government in Kabul and its forces stand on their own after most foreign troops depart in little more than two years.

On Sunday, a US official confirmed the latest death, saying that an international service member killed in an apparent insider attack by Afghan forces in the east of the country late Saturday was American. A civilian contractor with NATO and at least two Afghan soldiers also died in the attack, according to a coalition statement and Afghan provincial officials. The U.S. official spoke on condition of anonymity because the nationality of those killed had not been formally released. Names of the dead are usually released after their families or next-of-kin are notified, a process that can take several days. The nationality of the civilian was also not disclosed.

In addition to the 2,000 Americans killed since the Afghan war began on Oct. 7, 2001, at least 1,190 more coalition troops from other countries have also died, according to iCasualties.org, an independent organization that tracks the deaths.

According to the?Afghanistan?index kept by the Washington-based research center Brookings Institution, about 40 percent of the American deaths were caused by improvised explosive devices. The majority of those were after 2009, when President Barack Obama ordered a surge that sent in 33,000 additional troops to combat heightened Taliban activity. The surge brought the total number of American troops to 101,000, the peak for the entire war.

According to Brookings, hostile fire was the second most common cause of death, accounting for nearly 31 percent of Americans killed.

Tracking deaths of Afghan civilians is much more difficult. According to the U.N., 13,431 civilians were killed in the Afghan conflict between 2007, when the U.N. began keeping statistics, and the end of August. Going back to the U.S.-led invasion in 2001, most estimates put the number of Afghan civilian deaths in the war at more than 20,000.

NEW YORK???An Occupy Wall Street protester has lost his latest bid to prevent the Manhattan district attorney's office from using his tweets against him, clearing the way for the judge overseeing the case to unseal the tweets and give them to prosecutors.

In a case that has drawn the attention of electronic privacy advocates, a New York judge denied a request from the protester, Malcolm Harris, to put the tweets on ice while his appeal is pending.

Harris, 23, was arrested along with hundreds of others during a mass march across the Brooklyn Bridge in October 2011 for obstructing vehicular traffic.

Prosecutors subpoenaed Twitter for several months of his tweets, which are no longer available online, to combat his argument that police appeared to lead the protest onto the bridge before turning around and arresting them for walking on the roadway.

Electronic privacy advocates worry a precedent could be set that would make it more difficult for social media users to challenge law enforcement's use of their online words against them.

Twitter and Harris have unsuccessfully fought the subpoena, which has been repeatedly upheld as lawful by Manhattan Criminal Court Judge Matthew Sciarrino. The San Francisco-based company finally surrendered the tweets earlier this month after Sciarrino threatened it with contempt and a hefty fine.

Sciarrino had agreed to seal the tweets until a state Supreme Court judge ruled on Harris' latest request for a stay of Sciarrino's order upholding the subpoena. The judge, Carol Huff, denied the request this week.

Martin Stolar, Harris' attorney, said he was "disappointed" but would continue to pursue other legal challenges.

Sciarrino is reviewing the tweets and will provide to prosecutors those that appear relevant to the criminal case.

Twitter still has an appeal of Sciarrino's order pending in an intermediate appellate court, scheduled to be heard in November.

Harris also has a separate civil action pending, claiming Sciarrino overstepped his authority; prosecutors have asked a court to dismiss that lawsuit as an improper attempt to circumvent the normal appellate procedure.

Harris' criminal trial is scheduled for December.

The case is People v. Harris, Criminal Court of the City of New York, No. 2011NY080152.

Ithaca Estates Realty, a local rental company, has received final approval from the Town of Ithaca Planning Board to begin construction of a community shopping complex building project near the Ithaca College Circle Apartments this fall.

Evan Monkemeyer, real estate broker and community builder for Ithaca Estates Realty LLC, said the complex will house a fitness center, a bank and an Ithaca Bakery restaurant.

Monkemeyer said that he hopes a restaurant will rent one of the retail property and can use the over eighty outdoor dining spaces he has planned.

The project had been set to break ground in the summer of 2011, but it will now most likely start construction in early October. The complex should be ready to open by next fall, as long as they are able to obtain a building permit.

The project has been considered since around 2005, Monkemeyer said. The complex, which is set to hold both residential and commercial spaces, will be located on the corner of East King Road and 96B. There are also plans for a paved walkway that goes from the back of the building to the Circle Apartments.

College Crossings Ithaca — Shawn Steiner/The IthacanCollege Crossings, a community complex on East King Road and 96B, will include six commercial spaces and two four-or-five-person apartments.

Monkemeyer said financial reasons and difficulty obtaining bank loans forced him to postpone the project, which he estimates will cost him close to $2 million. The original design for the project was estimated to cost more, but Monkemeyer said he decided to contract it himself to cut costs. Monkemeyer said he doesn?t have projections about project gains yet.

In the original College Crossings design, Monkemeyer said, the building was supposed to be a commercial building with offices on top and businesses on the bottom. However, in the current design, the building, which will be more than 19,000 square feet, will have six commercial spaces on the main floor and a space for two four-or-five-person apartments. This makes it a mixed-use commercial and residential building.

Sue Ritter, director of planning for the Town of Ithaca, said the project seems to be on track.

?What?s required is getting site plan approval and special permits from the planning board to be able to have this mixed-use building,? Ritter said. ?The next step really is for them to come to the town with a building permit.?

One feature of the project that the Town of Ithaca planning board liked was the plan to add a paved walkway from the back side of the location to the college, Monkemeyer said. He also said the walkway will? begin at the College Crossings parking
lot and will? to the Circle Apartments, which will make it easier for students to commute to downtown.

?It?ll give you pedestrian access from East King Road, in the Town of Ithaca, all the way to downtown,? Monkemeyer said. ?It will be blacktop and smooth, so you can bicycle, skateboard or jog.?

Junior Ian Vitkus, a resident of the Circle Apartments, said he thinks the project has potential to be useful to the college community.

?It could be a place to get stuff that?s actually close to campus that you don?t have to drive to,? Vitkus said. ?Underclassmen don?t have to rely on the buses.?

Monkemeyer said he hopes the project will benefit the South Hill community. He said he hopes to turn it into a more popular destination.

?For the size of the project, I think it?s going to be Collegetown for South Hill,? Monkemeyer said. ?It?s not going to be College Avenue or Dryden Road, not with six story high-rises. This is going to be a suburban look, a suburban feel, and it?s going to be a landmark building.?

Charles Plosser, president of the Fed's Philadelphia branch, told an audience Tuesday that the Fed's effort to support the economy would likely fall short of its goals. And if the Fed looks ineffective, it could undermine future Fed action.

The speech probably startled some investors who had faith in the Fed's latest plan, said Jack Ablin, chief investment officer Harris Private Bank. The plan includes buying $40 billion in mortgage bonds each month until the economy improves.

"So many investors have bought into the illusion," he said. "And it was like Plosser pulled up the curtain on the Wizard of Oz."

The Standard & Poor's 500 index lost 15.30 points, its fourth straight decline, to close at 1,441.59. The 1.05 percent drop was the worst for the S&P since June 25.

The Dow Jones industrial average lost 101.37 points to close at 13,457.55. Caterpillar tugged the Dow down, losing 4 percent. The world's largest maker of bulldozers and other heavy equipment said late Monday that slower economic growth around the world dampened its earnings forecast. Its stock sank $3.86 to $87.01.

A batch of encouraging economic reports gave the stock market a nudge in morning trading. House prices rose in major cities for a third straight month, and a gauge of consumer confidence came in surprisingly high.

More surprising than those two economic reports was the Richmond Federal Reserve's strong reading on regional manufacturing, a recent trouble spot, said Phil Orlando, chief equity strategist at Federated Investors.

"Look at that. There were three data points on the economy and we crushed them," said Phil Orlando, chief equity strategist at Federated Investors.

But sagging profits could drag on the stock market in the coming weeks, Orlando said. Caterpillar joined a growing collection of companies have lowered their earnings forecasts. FedEx, a bellwether of world trade, said that shipping has sunk to recession-like levels. Railroad giant Norfolk Southern has also warned that falling shipments and sinking coal prices will likely drag down its earnings.

Wall?Street?analysts now estimate that corporate profits will sink this quarter from a year earlier. That would be the first such drop in three years.

The Nasdaq composite index dropped 43.05 points to 3,117.73. Google's stock touched an all-time high in early trading, clearing $764, but closed the trading day at $749.16.

Apple, the largest public company in the world, lost $17.25, or 2.5 percent, to close at $673.54. It has lost more than $26 in two days. Apple is the biggest component in the S&P but is not included in the Dow, helping explain why the S&P suffered a greater percentage decline than the Dow's 0.8 percent.

The closely watched Standard & Poor's/Case Shiller index of national house prices increased 1.2 percent in July compared with the same month in 2011. Prices rose from the previous month in all 20 major cities tracked by the report for the third month in a row.

The Conference Board said its gauge of consumer confidence shot to a seven-month high in September, trumping forecasts by a large margin. People surveyed said they were more optimistic about the job market.

The Federal Reserve's manufacturing index, which surveys companies in the central Atlantic region, increased after shrinking for three months as businesses turned more optimistic. Companies said they anticipate more orders and shipments even as employment dips. The index turned positive in September after a negative reading in August.

All three major stock indexes have surged or more this month, buoyed by a new plan from the Federal Reserve to support the U.S. economy. Both the Dow and the S&P 500, the benchmark for most stock funds, have gained more than 3 percent.

Treasury prices rose as traders shifted money into safe assets. The 10-year Treasury yield, the benchmark for mortgages and other loans, dipped to 1.67 percent, down from 1.71 percent late Monday.

But unlike Blood's Daniel Plainview and Eli Sunday, Freddie and Dodd aren't opposites, or even really enemies; although it's still very much a struggle for power, their relationship is a lot more complicated, with enough love and heartbreak for a dozen romantic dramas.

Dodd initially appears to be the one in command -- and the key to a safe, happy future for our anguished drifter -- but after our first few moments in his presence, we start getting hints that behind his self-made sect's glowing fa?ade (and his own charismatic persona) lurks a different kind of darkness, as well as another power struggle between the members of Dodd's family (led by his quietly terrifying wife Peggy, played to steely perfection by Amy Adams). As Roger Ebert put it, "The Master is fabulously well-acted and crafted, but when I reach for it, my hand closes on air."

Personally, I started off in the first camp, but as the movie wore on, I found myself edging into the third. It's certainly Anderson's chilliest movie; it sweeps you up while you're watching, but it always keeps you at a distance, so while you're trying to untangle its themes later on -- themes that Anderson seems to make a point out of refusing to fully address -- it's easy to wonder what the point of the whole thing really is. The Master lingers, but not in a particularly enjoyable way.

It can be argued that it's unfair to expect high art from a film, and Anderson himself insists that his movies are the stories of people, not themes or ideas -- which is certainly more than fair. But with The Master, Anderson the screenwriter fails Anderson the director -- the characters are just sketches and their actions are inscrutable, so if they aren't being used to tell a broader story, then they don't really serve any purpose at all. Sometimes it feels like we're seeing half a movie; the effect is unpleasant, unmooring.

But on the other hand, it's hard to look at The Master in the context of modern American cinema and come away judging it too harshly. As frustrating as its tendency to elide can be, it's still a film that refuses to talk down to its audience. (It may accomplish that by refusing to speak to its audience at all, but still.) It's beautiful in an era when true cinematography is hard to find, it's deeply ambiguous in a studio system that prizes the lowest common denominator, and it contains some of the most powerful acting you're liable to see all year. If it's a failure for Anderson, maybe it's still a victory for filmgoers who want to be treated like adults -- who want to consider and discuss what they've seen over a period of days and weeks, instead of forgetting the whole thing before they're even out of the theater parking lot.

On those terms, I appreciated The Master, and I'm glad Anderson is out there fighting the good fight, proving mainstream audiences still want something more than mindless entertainment -- but I wouldn't watch it again, despite the prevailing notion that it's a film that needs to be seen two or three times to truly appreciate. At one point in the movie, there's an exchange that goes as follows:

"I don't understand it."

"I don't either -- that's why we're here."

That about sums up the whole experience for me -- and fittingly, as I walked out of the theater, Rick Astley's "Never Gonna Give You Up" was playing on the PA system. I had to laugh; for film fans, The Master might be the biggest Rickroll of the year.

BRUSSELS (Reuters) - Vivendi's Universal Music Group clinched EU regulatory approval on Friday to buy EMI's recorded music unit for $1.9 billion after agreeing to sell record labels that bring in nearly a third of the British company's revenues.

Also on the block will be the Mute, Ensign and Chrysalis labels, EMI Classics, Virgin Classics, EMI's share of the "NOW! That's what I call music" compilation business, and EMI units in France, Spain, Belgium, Denmark, the Czech Republic, Poland, Portugal, Sweden and Norway.

Universal will also sell its brands Sanctuary, Co-Op Music Ltd, King Island Roxystar, MPS Records, its share in Jazzland, and its Greek unit.

Reuters reported on September 3 that the EU watchdog would clear the takeover after Universal offered to sell global rights to some of EMI's record labels and catalogues.

Industry experts said the value of the package could be as high as $750 million.

The European Commission said buyers must be active record firms or those with a proven track record in the music industry, ensuring that there would be a strong rival to Universal.

Potential buyers include Warner Music Group, BMG - which is the music publishing joint venture between German media group Bertelsmann and private equity group KKR, as well as Virgin Records founder Richard Branson and Sony Music.

"The very significant commitments proposed by Universal will ensure that competition in the music industry is preserved and that European consumers continue to enjoy all its benefits," EU Competition Commissioner Joaquin Almunia said in a statement.

EMI's seller, Citigroup Inc, acquired EMI from buyout group Terra Firma when it defaulted on loans owed to the investment bank. The proposed deal will cement Universal's No. 1 position in the European music industry.

Wednesday's polls also show voters in all three swing states believe Obama would do a better job than Romney handling an international crisis and national security, areas with added weight in light of the ongoing tension in the Middle East and North Africa.

The PollTracker Average currently shows Wisconsin and Virginia slightly favoring Obama, while Colorado is in tossup territory.

The swing state polls were conducted by Quinnipiac University Sept. 11-17 using live telephone interviews of 1,497 likely voters in Colorado, 1,474 likely voters in Virginia and 1,485 likely voters in Wisconsin. Each sample has a margin of error of three percentage points.

Windows 8 is coming folks, and so is an onslaught of new machines featuring Microsoft's something-for-everyone OS. Dell already showed us some of its fresh consumer Win8 hardware back at IFA 2012, and now it's the enterprise's turn to shine. First up is the Latitude 10 tablet, which packs an Intel Atom SoC, a 10.1-inch IPS 1366 x 768 LCD display covered in Gorilla Glass, 8-megapixel primary camera plus an HD front-facing shooter. It's got 2GB of RAM and up to 128GB of eMMC NAND storage, plus an SD card slot should the integrated storage prove insufficient. Connectivity comes via one full-size USB 2.0 port, a microUSB charging socket, mini-HDMI, a headphone/microphone combo jack, proprietary docking port and a micro-SIM slot for WWAN use. The Latitude 10 packs up to a 60Wh battery, which isn't remarkable in and of itself, but the fact that it's removable is. That means road warriors can travel with a spare cell or two to keep their slate in the juice no matter how long they work on it. While the swappable battery can keep the 10 from being tethered to an outlet, the dock Dell built for it ensures it'll have a stylish place to rest when it is. The dock expands the slate's connectivity with four USB 2.0 sockets, Gigabit Ethernet, HDMI and audio output.

Next is the Optiplex 9010 all-in-one desktop we saw earlier this year. It still has the same 23-inch, 1920 x 1080 panel on the front and vPro-equipped Ivy Bridge silicon lurking beneath -- the only change is the upgrade from Windows 7 to Windows 8. The Latitude 6430u is an addition to Dell's venerable business laptop line, and is the first to bear the Ultrabook moniker. It's generous to label the 6430u as such, as it's .82 inches thick and weighs 3.7 lbs, but it's still a fairly thin and light laptop -- plus it has the same solid magnesium chassis construction as its Latitude brethren. The 6430u crams a 14-inch, 1366 x 768 matte display into its 13.3-inch chassis, and users have the option of Ivy Bridge Core i3, i5 and i7 silicon with vPro, up to 8GB of RAM and up to 256GB worth of solid state storage. Naturally, there's 802.11 b/g/n WiFi, Bluetooth and mobile broadband available for wireless connectivity. Unfortunately, we can't tell you how much Dell's new business computers will cost, but we do know that they'll be available when Windows 8 is, which is to say late October.
